Cancellation Policy
Please note if you fail to cancel an appointment without at least 24 hours notice (business days), you are responsible for the full fee of the session unless you are able to reschedule the same week or make some other mutually agreed upon arrangement. Insurance companies do not reimburse for missed sessions. Actual emergencies are excluded from this policy. Our cancellation policy is standard practice for the field.

Forms can be downloaded and completed prior to the first session.
Otherwise please arrive 15 minutes early for your first appointment to complete them.
